Name, Phone Number and Email Address needs to be the contact information of the guardian that will be granting the child permission to play for Newport Firefighters.

Street Address, City, State and Zip need to match where the child actually lives.

Child′s Full Name needs to be entered including their middle name.

Child Full Birthdate needs to include the year, so we can know which division they will play/cheer in.

Child′s Sport can be selected with clicking the checkbox next to the correct one.

Notes are optional if your wanting to pass on any information or ask any questions.  This will also be a perfect spot to notify us if you believe your child will play "above" thier age division due to the weight restrictions.
